Road where the hazard occurs:	Please remember to use state or federal road name. (I-75, US-25, KY-17, etc.) and county where hazard occurs. District 6 only maintains state and federal roads.
Specific location of hazard:	Please be as specific as possible when describing the location of the road hazard. County, mile markers, closest intersection, road direction (north, south, east, west). Example: The pothole is in the right lane of I-75 North just south of mile marker 177.
